Parents and athletes,

This is a reminder that it is necessary for all athletes to have a current physical to participate in UHSAA athletic events. UHSAA and Jordan School District require that this physical must be dated between June 1st and May 31st of the participating year. They also require this to be a complete physical and NOT a wellness check. All physicals need to be turned in by March 8th for spring sports. Please upload your completed physicals to the website.
If you cannot get into your doctor before then, we will have Registered Physical Therapist (RPT) here at the school on Thursday, February 25th, for all athletes.
Because of Covid-19 we need to do the physicals in the following manner. Please follow directions carefully to keep everyone safe.
Athletes will line up outside of the high school gym and enter through the Southeast door. Please be sure to social distance while you wait outside the doors.
It will also be required to have a mask and your temperature checked to enter the gymnasium for your physical.
We are asking seniors to come between 5:00 pm. – 5:25 pm
Juniors between 5:25 pm. – 5:50 pm
Sophomore between 5:50 pm – 6:15 pm
and Freshman from 6:15 pm – 6:40 pm
If you cannot make your time slot, we will accept stragglers from 6:40 – 7:00 pm. The doors will be closed by 7:00 pm, so that the doctors can be finished by 7:15 pm.
These physicals will cost $25. Checks can be made out to RPT. Please remember to download UHSAA physical form and bring it with you. Parents must sign page 2 and 3 in order for the doctors to examine your child.
Once again, completed physical forms need to be uploaded to before they are eligible to participate.
